What it means

When a company is going sideways, they stop. They’ve decided the risk of raising issues isn’t worth the return. They’re protecting themselves.
The cascade
Once your best people stop raising issues, you lose your early warning system. Problems grow in the dark. By the time they surface, they’re twice as expensive and half as fixable. The company is now in a quiet crisis the CEO doesn’t see.
The fix
Go ask your top five people, individually, what they’re worried about that they haven’t told you. Listen. Don’t defend. Don’t explain. Just listen. If the answers surprise you, that’s your signal. You’re closer to a turnaround than you realize.
